Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 18, 2019 from 11:05 a.m. through 1:30 p.m. Five children records were reviewed. The records for the provider and two household members were reviewed and updated. There were 10 children in care along with the provider and assistant for points totaling thirty-one. No medication is being administered at the present. Areas of non-compliance may be found on the violation notice.
Areas observed; emergency supplies were inspected, first aid kit, required postings were observed, lunch preparation, lunch, hand washing/diapering and bathroom procedures, and nap. Interactions between the children and staff were observed as positive and the children seemed comfortable with the daily routine and schedule.

Based on record review the provider failed to obtain an updated rabies vaccination for one pet.
Evidence: 1. Four pet records were reviewed. 2. One pet had not obtained updated rabies vaccination. 3. Last documentation of rabies vaccination was dated to be completed by 11/5/2019. 4. The provider confirmed the rabies vaccination had not been obtained.
Plan of Correction:
The rabies vaccination is scheduled for November 22, 2019.
