Comments:
The licensing inspector conducted an unannounced renewal inspection, at which play groups, outside time, and pre-lunch activities were observed. Children were observed; and staff and children files and other documentation were reviewed. Three (3) children were found to be in care, aged 4mths-5yrs. Based on review of files and discussions with the provider, the provider failed to maintain compliance with this standard related to obtaining background checks for a person residing in the home as required.
Evidence: There was no documentation that the Finger-print Check or Central Registry Clearance was completed on household member #2 as required. The household member moved back into the home in October 2021. The allowed 30 day time frame to obtain these has expired.
Plan of Correction:
Both required background checks will be completed immediately and licensing notified when the findings of these is received.

Description:
Based on review of files and discussions with the provider, the provider failed to maintain compliance with this standard as required related to proof of birth documentation.
Evidence: Child records #1 and #3 did not have documentation of verification of birth as required. Both children had been in care longer than the allowed 7 days to obtain this verification.
Plan of Correction:
Proof of birth will be recorded within the children's files as required.

Description:
Based on review of files and discussion with the provider, the provider failed to maintain compliance with this standard as required related to CPR certification.
Evidence: The provider did not have current certification in CPR. The previous certification held expired in 2020.

Provider will complete a CPR/FA approved course as soon as possible. Provider will notify licensing as soon as completed.

Description:

8VAC20-800-170-A
Description:
Based on review of files and discussions with the provider, the provider failed to maintain compliance with this standard related to PPD/TB testing of household members as required.
Evidence: There was no PPD/TB testing results for household member #2 as required. The individual has been within the home since October 2021. Testing is required to be completed within 30 days of becoming a household member.
Plan of Correction:
A TB/PPD test will be completed and results placed in the file. Licensing will be notified upon completion.
