Comments:
A mandated monitoring inspection was conducted on 10/4/2023 from 1:22 pm to 2:35 pm. Two children were in care at the time under the care of one caregiver. The Licensing Inspector reviewed two children?s records, one staff record, and other program records. The LI conducted a walk-through of the house.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and interview, the family day home failed to obtain written permission to take a child off the premises for regularly scheduled trips.
EVIDENCE: The record for child 1 has no documentation that the parent has given permission for the child to be taken off the premises. An interview with staff 1 reveals that children in care regularly go on neighborhood walks.
Plan of Correction:
Prepare doc. in advance when planning a field trip.
