Comments:
An unannounced code compliance inspection was conducted on 10/12/2023 from 10:00AM to 12:25PM. At the time of the inspection there were 27 preschool children in care and 6 staff members. Children were observed participating in table and floor activities, center play, hand washing and eating snack. The inspector reviewed 10 staff member records and 5 five children?s records. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
Based upon review of records and staff interview, it was determined the facility has not ensured that each person in a supervisory position has been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision. Evidence: 1. There was no physician statement available for staff 2 2. Staff #7 acknowledged that there was no physician statement for staff 2.

Description:
Based on observation and interview, it was determined the facility did not ensure there was cushioning material under playground equipment. Evidence: 1. The cushioning material in front of the slide chute had been pushed away exposing the hard ground underneath. 2. Staff #7 confirmed the cushioning material at the exit of the slide chute had been pushed away.

Description:
Based on a review of 10 staff records, it was determined that the vendor did not ensure that all staff who had lived out of state within the past five years had a criminal record check and a central registry check on file from the state the staff member had lived. Evidence: 1. Staff #2 indicated on her sworn statement that she had lived out of state in the past five years. There was not a criminal history check or a search of the central registry on file from that state. 2. Staff #8 indicated on his sworn statement that he had lived out of state in the past five years. The state is considered an "NFF" state, so criminal history check was not required. However, there were no central registry findings on file from that state. 3. Staff #10 indicated on her sworn statement that she had lived out of state in the past five years. There was not a criminal history check or a search of the central registry on file from that state.
