12/6/19 Arrived 2:00pm Departed 3:20pm An unannounced monitoring inspection was conducted today. There are 8 children in care totaling 23 points with two caregivers. The children were resting. Based on observation, the facility failed to ensure that children birth through 12 months of age sleep in a crib. Evidence: 1. During the inspection, child #3 (age six months) was sleeping on the sofa.
