9/5/23 Arrived 9:25am Departed 10:10am A subsidy health and safety (SHSI) inspection was completed. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection revealed one repeat violation with applicable standards or law. A violation notice was issued.
Based on a review of staff records, the facility failed to ensure that employees complete the Virginia Preservice Training within 90 days of employment. Evidence: 1. Staff #1 has not completed the Virginia Preservice Training. Hire date 4-1-22 REPEAT VIOLATION
Plan of Correction:
Staff #1 reported being near completion of the training.
