Comments:
An unannounced code compliance inspection was conducted on-site on November 1, 2023. There were 6 children present with 2 staff supervising. All areas of the facility including classrooms, hallways, outdoor play area, and bathrooms were inspected. The inspector reviewed religious exempt requirements, 5 children's records and 5 staff records.
Information gathered during the inspection determined non-compliance with applicable laws and violations were documented on the violation notice issued to the program.
Based on a review of children's records and interview, the center failed to ensure all children in the center are in compliance with immunization provisions. Evidence: The record of child 1 did not contain documentation of immunizations. Management confirmed there were no immunizations in the file.

Immunization records will be obtained.

Description:
Based on observation and interview, the center failed to ensure all areas of the premises accessible to children are free of obvious injury hazards to include maintaining cushioning material under playground equipment. Evidence: 1. There was a container of Clorox disinfectant wipes on a table accessible to children. Staff confirmed the wipes were kept on the table. 2. On the playground there was no cushioning material under the fireman's pole which was more than 5ft tall. Staff confirmed there was no cushioning material under the pole area.
Plan of Correction:
Clorox wipes are now stored out of reach of children. Pea gravel will be added to the low cushion areas.

Description:
Based on a review of staff records and interview, the center failed to obtain repeat background checks every 5 years. Evidence: The record of staff 1, employed in 2009, contained a fingerprint check that expired 09/13/18. Management confirmed the repeat check was not completed.

Staff completed a fingerprint appointment on 11/07/23.

Description:
Based on a review of staff records and interview, the center did not ensure one staff had the results of the central registry check within 30 days of the individual beginning employment. Evidence: The record of staff 5, employed in 2022, did not contain documentation of central registry results. Management confirmed the results were not in the file.
Plan of Correction:
Staff completed the digital background check on 11/06/23. We're waiting on CRS to send us results.
