Comments:
A renewal inspection was conducted on April 29, 2024 between 10:27am and 12:05pm. There were 6 children present, with 2 caregivers supervising. A total of 5 child records, 2 caregivers records and 4 household members records were reviewed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, nutrition, handwashing, outdoor play area, medication, special care and emergencies. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the family day home did not obtain documentation of additional immunization for a child once every six months for children under the age of two years. Evidence: 1. The record for Child #1 (DOB 05/2021) did not have additional immunizations. Last update was 12/06/22.
Plan of Correction:
I will contact the parent now so she can bring the updated immunization record.
