Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 02/03/2021 and concluded on 02/04/2021. The director was contacted by telephone to initiate the inspection. There were 31 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. If you have any questions regarding this inspection, please contact the Licensing Inspector, Maria Robles at maria.robles-lopez@dss.virginia.gov. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request and an out-of-state Criminal History Name Check. Evidence: 1) The records for Staff #1 and Staff #2 did not contain documentation of out-of-state Sex Offender Registry search requests and out-of-state Criminal History Name Checks. Staff #1 and #2 had declared in their sworn statements having lived out of the state of VA within the last 5 years before employment. 2) The program director stated that she was not aware of the new out-of-state background checks requirements.
Plan of Correction:
I will send out the out-of-state background checks requests immediately.
