Comments:
An unannounced monitoring inspection was conducted on 9/19/2022 from 11:30 am-3:00 pm. There were 35 children supervised by 5 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on observation, areas of the center, outside, were not maintained in a safe and operable condition.
Evidence: 1. 2 out of 2 outdoor playsets located on the preschool playground were cracked, creating a hole at the bottom of each playset.
