Comments:
An announced monitoring inspection was conducted with the provider beginning at 1:00 PM and ending at 1:45 PM. There were 4 children, age 2 through 5 years old, in care of the provider for a total of 7 points. The children were observed interacting with the provider and playing with age appropriate toys. Four children?s records were reviewed. Three household records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and battery-operated radio were reviewed. The most recent emergency evacuation drill was documented as July 8, 2022. The most recent shelter-in-place was documented as April 11, 2022.

Based on review of 5 children's record, the provider did not obtain ensure that the parents review the emergency contact information and obtain the parent's signed acknowledgment of the review. Evidence: Child A, child B, child C, and child D's record did not have an annual signature to acknowledge review.
Plan of Correction:
The provider stated that she would have to parents review the emergency contact information and sign the documentation.
