Comments:
An unannounced renewal inspection was conducted with the provider on January 6, 2023 beginning at 10:35 AM and ending at 11:55 AM. There were four children, ages 22 months, 3, 3, and 3 years old for a total of 9 points. The children were observed doing an alphabet puzzle on the floor with the provider. The children were also observed building a log cabin with wooden blocks. Four children's records were reviewed. The provider and two household members' records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent practice emergency escape drill was documented as 12/06/2022. The most recent practice shelter-in-place drill was documented as 8/16/2022.

Based on a review of four children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child C (start date 2/22/22) and child D's (start date 6/6/22) record did not have documentation of a physical examination in them.

The provider stated that she would ask the parents for the missing information and will put it in their records once it is received.

Description:
Based on a review of four children's records, the provider did not have documentation that the parents received the policies for the administration of medication. Evidence: Child A, B, C, and D's records were missing documentation that the parents received the policy regarding administration of medication from the provider.
Plan of Correction:
The provider will print a copy of the sample form, provide the policy in writing to the parents, and place documentation that they received the policy in each child's record.
