Comments:
A Subsidy Health and Safety inspection was conducted on December 4, 2019 to review supplemental health and safety requirements. Three staff were present with twelve children. Five children and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-580-E-2
Description: Based on review, facility did not ensure 2 of 5 staff are certified in first aid within 90 days of beginning employment.

Evidence: The records for staff #3 (start date January 3, 2019) and staff #4 (start date September 3, 2019) did not have documentation of first aid certification.

Plan of Correction: Director will explore possible training for staff.

Description: Based on review, facility did not ensure all required policies and procedures are developed and written.

Evidence: There was not a written policy and procedure that describes how the facility will ensure that each group of children receives care by consistent staff or team of staff members.

Plan of Correction: Policy was written and posted during inspection.

Standard #: 22VAC40-665-770-B-5
Description: Based on review, facility did not ensure the written emergency preparedness plan has documentation of required procedural components.

Evidence: The emergency preparedness plan did not have continuity of operations procedures to ensure that essential functions are maintained during an emergency.

Plan of Correction: Policy was written during inspection and posted.
