Comments:
An unannounced monitoring inspection was conducted on 6/21/2023. There were 9 children, ages 1 year ? 11 years, totaling 16 points and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records were reviewed. The children were observed during center time. The inspector arrived for the inspection at 9:41am and departed at 11:16am.
Discussion on standards related fire safety and shock protection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of four children?s records, the provider failed to ensure that each immunization record shall be signed by a licensed physician, the physician?s designee , or an official of a local health department.
Evidence: Child 3 start date was 12/27/2016 and on the day of the inspection the immunization record was not signed by a physician.
Plan of Correction:
Provider will obtain a signed immunization record from parent.
