Comments:
An unannounced monitoring inspection was conducted 12/7/2023 from 5:20pm through 5:50pm. There were 14 children directly supervised by 3 staff. Off site staff file review was conducted 12/19/2023. The physical plant, programming, menu, 3 staff records, 5 children?s records, 2 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing basketball and other games in the gym. Positive interactions between children and staff were observed during this inspection. There was an adequate supply of age appropriate materials for the children. The center was clean and organized. All staff present at the time of inspection had current CPR/First Aid and Daily Health Observation certification. Two staff present had current MAT certification. Areas of non-compliance are identified in the Violation Notice.

Description: Based on medication review and record review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: Child #7 is allergic to peanuts and has an emergency medication on site. The center does not have a written allergy care plan signed by the child's doctor on file.
