Provided consultation on standard 150: Form and content of immunizations and physical examination - Each report shall include the date of the physical and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 9/14/2020 and concluded on 9/15/2020. The director was contacted by telephone to initiate the inspection. There were seven children present with two staff supervising. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children's records, two staff records, the emergency drill log, the fire and health inspection and staff MAT, DHO and CPR/First Aid certification to ensure documentation was complete. The Inspector also viewed the contents of the first aid kit along with the battery operated flashlight to ensure compliance. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
