Comments:
An unannounced monitoring inspection was conducted 1/22/2024 from 8:45am through 10:00am. There were 18 children directly supervised by 2 staff. The physical plant, programming, menus, 2 staff records, 5 children?s records, 2 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing uno, jenga, reading books, and playing sharks and minnows. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not maintain and keep at the center a separate record for each enrolled child that contains all required information.
Evidence: Child #1, Child #2, Child #3, Child #4, and Child #5's records did not contain the following required parental agreements: authorization for emergency medical care, statement that the center will notify the parent when the child becomes ill and the parent will arrange for the child to be picked up as soon as possible, and a statement that the parent will inform the center within 24 hours or the next business day after the child or any member of the immediate household has developed any reportable communicable disease.

Based on documentation review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the even of a suspected or confirmed allergic reaction.
