The licensing inspector completed an inspection in response to a self-reported concern regarding forbidden action that occurred at the center on 10/17/2019. The Department was notified of the incident on 10/21/2019. The investigation was initiated on 10/25/2019 and completed on 01/15/2020. On 11/01/2019, the licensing inspector was on site from approximately 12:00 PM until 2:10 PM interviewing staff, reviewing documentation, and conducting observations. Additional interviews were conducted on 10/25/2019, 11/08/2019, 11/12/2019, 11/13/2019, 12/05/2019, and 01/08/2020. There were no violations of standards found during the inspection.
