Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:00 AM and departed at 1:30 PM Morning activities and outdoor play were observed. A sample size of five staff records and six children's records were reviewed. Medication administration and injury records were reviewed.
An exit meeting to review the acknowledgement form was conducted with the program director prior to closure of the inspection.
Based upon observation and staff interview, the facility has not ensured that all areas are safe. Evidence: 1. There was a pair of sharp adult scissors is an unlocked drawer in a cabinet in the studio. 2. The child safety clasp on a drawer under the counter in the Sea Turtles room did not catch when the drawer was opened. There was a pair of adult scissors and stick pins in the drawer. 3. Staff 1 verified the above.
Plan of Correction:
The facility responded with the following: The safety clasp on the drawer in the Sea Turtles was repaired during the inspection. The adult scissors in the studio were moved to an inaccessible location. All staff will be reminded to be diligent when checking for safety issues.

Description:
Based upon observation and staff interview, the facility has not ensured that pillows are covered with cases and assigned for individual use. Evidence: 1. There were pillows in cubbies outside of the studio that did not have a cases and were not labeled with names for individual use. 2. Staff 1 verified that some pillows were not labeled with names and did not have covers.
Plan of Correction:
The facility responded with the following: Staff were reminded of the requirements regarding pillows during the inspection. All pillows will have names and removable cases.
