Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector was in the facility between 9:30 AM and 12:30 PM. Seventy-eight children were in care. Morning activities, outdoor play and snack service were observed. A sample size of seven children's records and five staff records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted with the program director prior to closure of the inspection.
Based upon observation and staff interview, the facility has not ensured that potentially hazardous substances are kept in locked locations using a safe locking method that prevents access by the children. Evidence: 1. There were two containers of shaving cream, labeled "caution, keep out of reach of children" in an unlocked cabinet under the counter in room 202. 2. Staff 1 verified that the containers of shaving cream had warning labels and were not in a locked location.
Plan of Correction:
The facility responded with the following: The shaving cream was immediately moved to a locked cabinet. All staff will be reminded that shaving cream must be kept in a locked cabinet when not being directly supervised by staff.

Description:
Based upon observation and staff interview, the facility has not ensured that pesticides are not stored in areas used by children. Evidence: 1. There was a small tray of Terro Ant Killer on the counter in room 303. The ant killer has a hazard warning label. 2. Staff 1 verified that the ant pesticide was on the counter.
Plan of Correction:
The facility responded with the following: The ant pesticide was removed to a locked cabinet during the inspection and will be removed to a storage area in an area not used by children. All staff will be reminded of the appropriate use and storage of pesticides.
