Comments:
The licensing inspector conducted an unannounced complaint inspection on July 17, 2019 in response to a complaint that was received by the Department on June 19, 2019 concerning the care and supervision of children. The inspector was on site on July 17, 2019 from 1:05pm to approximately 3:40pm. Interviews were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the Violation Notice for more details.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on a review of documentation and interviews on 07/17/2019, the center did not ensure that when staff were supervising children, they shall always ensure their care and protection.

Evidence: 1) Child #1 (five-month-old) was in the care of Staff #1 in the afternoon of 06/12/2019. During interview, Staff #1 reported that on 06/12/2019, she fed Child # 1 with the infant formula provided by the center instead of the infant formula provided by Child #1?s parent. Staff #1 reported she was aware that Child #1 had a special type of formula at the center due to possible stomach issues, but fed Child #1 the center?s formula because she thought someone had tampered with Child #1?s formula. Staff #1 reported the can of formula had writing on it with instructions on how to prepare Child #1's bottle, but she had not "seen it before" and did not feel it was safe. Staff #1 reported she did not report her concerns about Child #1's formula to management and did not consult with the parent prior to feeding Child #1 a different type of formula.

Plan of Correction: Per the Center:
"1.) Staff should consult management/administration anytime there are questions or concerns in regards to the children in their care. If a situation presents itself the administration will be responsible for contacting the child's parent(s) immediately.

2.) To prevent this non-compliance from reoccurring we implemented a communication/inventory log into our Infant/Toddler class, so that all staff members can effectively document the supplies brought in by the parent each day. The administration will now require all parents to notify us anytime there is a change to the care that we are accustomed to providing their child specifically.

3.) The administrative staff which consists of the Executive Director, the Assistant Director, and the office assistant will be the party(s) responsible for monitoring the preventative measures and seeing that each step is correctly implemented into daily function."
