Comments:
The licensing inspector conducted an unannounced inspection in response to a self-reported concern regarding forbidden action by staff. The Department was notified of the incident on November 8, 2019. The inspection was initiated on November 8, 2019 and completed on January 10, 2020. On November 19, 2019, the licensing inspector conducted an unannounced joint inspection with local agencies from 1:00pm until approximately 2:43pm. Interviews were completed, documentation was reviewed, and observations were made at the center. There were no violations found related to the self-report. However, an unrelated violation was found and is identified on the Violation Notice.

Please complete the `plan of correction? and `date to be corrected? for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 22VAC40-185-520-C
Description: Based on a review of records, observation, and interviews on 11/19/2019, the center did not ensure that all requirements were met for the use of diaper cream.

Evidence: 1) The licensing inspector observed diaper cream at the center for Child #1 (DOE: 11/02/2018).

2) During interview, staff reported the diaper cream had been applied to Child #1 on several occasions since Child #1 was enrolled at the center. However, a record of the diaper cream use and adverse reactions was not kept on file for Child #1.

3) A non-prescription over-the-counter skin products authorization form observed in the record for Child #1 did not have written parent authorization. A member of management reported a written parent authorization for the use of the diaper cream could not be located for Child #1.

Plan of Correction: Per the Center: "Staff will make sure that the parent signs the written consent form. The form will be kept in a notebook in the classroom. The notebook will be monitored by administration."
