Comments:
An unannounced monitoring inspection was conducted onsite on 9/12/2023 from 2:38 pm to 3:30 pm, staff records were reviewed in the main office on 10/4/2023 from 10:15 am to 12:00 am. There were 13 children in care, ages five years to ten years, in one group under the care of two staff. The Licensing Inspector reviewed three children?s records, three staff records, and other program records. The children were observed doing homework and playing.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the center failed to put in children's records the name, phone numbers, and addresses of tow designated people to call in an emergency if a parent cannot be reached.
EVIDENCE: 1. The record for child 1 listed the emergency contacts as the parents.
2. The record for child 2 listed as an emergency contact one of the parents.
Plan of Correction:
The center will obtain the correct information and update the registration form.
