Technical Assistance:
Reminded the provider that all cribs shall be spaced at least 12 inches apart when occupied.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 8 children, totaling 25 points. Children's records, caregiver/adult records, background checks, health records, caregiver training, emergency preparedness items, administrative documents, the physical plant, caregiver:child interactions, and programming were inspected today. The provider reported that no children's medications were on site today.

The children were observed playing outside, playing with magnet toys, interacting with both caregivers, and eating lunch. The children were served chicken strips, french fries, corn, mixed fruit, and milk for lunch today.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on a review of emergency preparedness items, it was determined that the provider has not completed an annual review of the emergency preparedness plan.
Evidence:
The provider most recently reviewed the emergency plan on 6/1/18.

Plan of Correction: I will review the plan with my assistant this week.

Standard #: 22VAC40-111-920
Description: Based on a review of administrative documents, it was determined that the provider has not updated the menu as required.
Evidence:
The provider did not serve the lunch foods indicated on her posted menu today. She did not update the menu to reflect these changes at least 24 hours in advance.

Plan of Correction: I will remember to update the menu.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete.
Evidence:
1 - Child records A and F do not include documentation of an annual review by the parent.
2 - Child record C does not include the parent's medical insurance policy number.

Plan of Correction: Each parent will be asked for the information.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the home has not been maintained in a clean, safe condition as required.
Evidence:
There is a section of peeling paint on the wall trim in the children's play area. The peeling paint section is on the lower portion of the wooden trim where it is accessible to the children.

Plan of Correction: We will fix the peeling paint this weekend.
