Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 2/11/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present and 2 caregiver staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on a review of children's records, it was determined that two records do not contain all required information. Evidence: Child record B does not include documentation showing the child's first day of attendance. Child record C does not include documentation showing the child's first day of attendance; each parent's full identifying information and contact information. The parent has listed the mother's information in both the mother and father information sections.
Plan of Correction:
I will update the date of enrollment. I will ask the parent to correct the registration form.
