Based on record review and discussion with the staff, the facility failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening/test no later than 21 days after employment.
Evidence: The record for Staff #2 did not contain documentation of a negative tuberculosis screening/test; the documented hire date for Staff #2 was 03/29/2019.
Plan of Correction:
Moving forward we will obtain tb screening within the 21 day time frame.

Description:
Based on record review and discussion with staff, the facility failed to obtain a follow-up tuberculosis screening/test at least every two years from the date of the first initial screening/test for each staff.
Evidence: The most recent tuberculosis screening/test in the record for Staff #1 was dated 08/16/2017; there was no follow-up screening/test for 2019 in the record for Staff #1.
Plan of Correction:
A test has been scheduled for 09/30/2019.
