Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 9/15/2020 and concluded on 9/28/2020. The director was contacted by telephone to initiate the inspection. There were 40 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on review of four children's records, the center failed to ensure that all required items were obtained.
Evidence: 1. Child # 2 did not two emergency contacts, did not have the phone number for the physician, and did not note any allergies. 2. Child # 4 was missing the father information, and physician's phone number.
Plan of Correction:
The program director will have the parents to complete all the missing information. All children's records will be reviewed and updated as needed.

Description:
Based on review of the new board officers, the center failed to ensure that all background checks were complete.
Evidence: 1. Board officer 2 and 3 took office in January 2020, a CPS check as not requested and or returned. 2. Board officer 2 and 3 did not have a completed Sworn Statement.
Plan of Correction:
The administration office has sent the CPS checks in and will have the board officers to complete a sworn statement ASAP.
