Comments:
An unannounced monitoring inspection was conducted today from 10:00am to 12:30pm with the Director. There were 26 children in care, ranging from 21 months to six years of age, supervised by five staff. The children were observed cleaning the classroom with soap and water, playing with musical instruments, reading the book "Chipmunk at Hallow Tree Lane", discussing fractions while making monkey bread for snack time, and doing puzzles. Five children?s records and three staff records were reviewed. The Center has staff with current certification in CPR and First Aid, and Daily Health Observation training. One medication and authorization form were reviewed and were in compliance and the center has staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The most recent Fire Inspection documentation was dated 9/13/19 and the most recent Health Inspection documentation was dated 8/27/19. Snack today was bread made by the children, snap peas, and hummus. The playground was not inspected due to rain. Based on review of three staff records, the center did not obtain all of the required documentation for staff records. Evidence: The record for Staff A, with a hire date of 9/9/16, did not contain documentation of two references as to character and reputation as well as competency before employment.

Description:
Based on observation, not all areas of the center were maintained in a safe condition. Evidence: In the bathroom of the North Star class there is a door that has splintering and peeling wood at the bottom, top, and sides that poses a scratching hazard.
Plan of Correction:
We will have the maintenance person come out to look at the door to see if he can fix it, and if not, then replace it.
