An unannounced renewal inspection was conducted on March 2, 2022. There was one classroom observed with 15 children, directly supervised by 1 staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed , arriving for the day, buildings with blocks, playing board games, and making crafts. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review, 2 of 4 children's records reviewed did not contain all required information. Evidence: 1. Child 1's record did not contain the name, address, or phone numbers for either of the child's guardians. 2. Child 4's record did not contain the address for two emergency contacts.
