Comments:
A monitoring inspection was conducted on, May 18, 2023. The inspector was on site from approximately 3:00 pm until 5:00 pm. The director was on site and assisted with the inspection. There were 16 children present, ranging in ages from six years to ten years, with three staff supervising. Children were observed having outside time, playing games in the gym and later had a party with cupcakes and ice cream. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

Based on a review of four staff records, the center did not obtain the results of an Out of State Sex Offender Registry completed for each state an individual has resided in the past 5 years prior to the 1st day of employment/service.
Evidence:
The record for Staff #4, employed 04/27/2023, indicated the staff had resided in another state outside of Virginia within the last five years. The record did not contain the results of an out of state sex offender registry.
Plan of Correction:
Per the director: I will complete an out of state sex offender registry search for the staff member.

Description:
Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Multiple cleaning materials including floor cleaner, sanitizer, and window cleaner were observed sitting on the counter in the cafeteria where children were in care.
Plan of Correction:
Per the director: I will talk with the school and ask that they lock up the cleaning materials before we start our program each day in the cafeteria.
