Technical Assistance:
Discussed resources to obtain renewal for medication training.
Discussed emergency preparedness plan and staff training.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 5 children in care with 3 staff present. When the Inspector arrived, the children were busy individually with morning activities such as table top activities, books and interacting with staff. Diapering and hand washing procedures were observed. The older children transitioned to the activity table where they made construction paper holiday trees. Infants were observed being held by staff, offered tummy time and observing the older children playing nearby. The home was decorated both inside and out with holiday decorations. A total of 3 children's records, all staff and one household record were reviewed. No medications on site at this time. Preparation for lunch was in progress. Previous violation from the March inspection was reviewed, and corrective action verified. This inspection began at approximately 9:50am and ended at 11:30am. Thank you for your cooperation today. Standard #: 22VAC40-111-960-E
Description: Based on observation of the refrigerator, infant bottles were not properly labeled.
Evidence: Infant bottles located in the refrigerator were not labeled with today's date.

Plan of Correction: We will request that the parent label all bottles with the child's name and also date.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, subsequent immunizations for one child was not obtained.
Evidence: Child #3's last documented immunizations were dated 3/19. Documentation of updated immunizations are required every six months for children under the age of two years.

Plan of Correction: I will request this documentation today from the parent.
