Technical Assistance:
As discussed, a model form for a TB risk assessment will be emailed to the provider.
Comments:
An unannounced Renewal inspection was conducted today. A total of 8 children (19pts) with the Provider and 2 Assistants were onsite today. The children were observed either playing with legos and toys or sitting at the table with staff completing an art project which involved coloring an outline of a cat and gluing newspaper onto the drawing. Once the projects were complete, they were hung up to dry. The home was decorated with a birthday board that had colorful balloons with the children's birthdays on them. Observation of both levels of the home was made that included observation of all cribs onsite. Cribs located on the upper level of the home are not currently in use. The bathroom and kitchen/food prep area was also observed along with the outdoor play equipment. The children transitioned to watch a television program before lunch. Lunch today was homemade pizza. A total of 6 children's files, all 3 staff files and both household member files were reviewed. Two violations found today. A corrective action for each was provided. Based on staff record review, one background check was not obtained. Evidence: Fingerprint search results were not obtained every five years for Staff #3. Last documented fingerprint clearance is dated in February 2019.
Plan of Correction:
The staff will schedule and complete the fingerprint search and the results will be forwarded to the Licensing Inspector within 10 days.

Description:
Based on child record review, parent authorizations for topical skin products were not obtained annually. Evidence: The parent authorization for a topical ointment for child #2 was dated 01/28/2023. The parent authorization for a topical ointment for child #5 was dated 03/11/2023.
Plan of Correction:
These ointments are no longer used. Both will either be returned to the parent or disposed of.
