Comments:
The Licensing Inspector initiated and completed an unannounced monitoring inspection on 5/22/2023. The inspector was on site from 10:10am until 12:55pm.
There were 51 children under the care of 10 staff. The ages of the children ranged from eighteen months to five years old. The children were observed during indoor and outdoor free play and nap time. Five children?s record and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to have documentation of the TB screening submitted at the time of employment and prior to coming into contact with children.
Evidence: Staff #2, which was hired on 6/22/2022 had record of a TB test/ screening dated for 7/6/2022.
Plan of Correction:
All staff will have Tb test prior to hire moving forward
