Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:00pm. There were 11 children (30 points) directly supervised by 2 staff. The home, 3 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. Hand washing procedures were also observed. There was an adequate supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice.

Based on review of emergency supplies, not all emergency supplies were in working condition.
Evidence: There provider was unable to turn on the flashlight and the portable battery-operated weather band radio observed on-site. There were no additional batteries available during the inspection to verify that the weather band radio and the flashlight were in working condition.

I will locate the batteries I purchased today.

Evidence: Child#2's record is missing the father's place of employment's name, address, and telephone number.

Description:
Based on record review, the provider did not obtain central registry results within 30 days of employment for a staff member.
Evidence: Staff#2's record does not contain central registry results. The staff member's first day of employment was documented as 9/2/2019.
