Comments:
A Subsidy Health and Safety inspection was conducted on 11/21/2019 to review supplemental health and safety requirements. There were 65 children, ages 5 years-11 years, and 7 staff members present during the inspection. The inspector reviewed 7 children's records, the on-site staff record requirements for 7 staff members and 3 medications during the inspection. The inspector reviewed the emergency supplies during the inspection. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are noted on the Violation Notice. Description: Based on record review and discussion with staff, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child #1 contained documentation of a diagnosed food allergy. The record for Child #1 did not contain a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The parent was asked to get an allergy care plan.
