Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.
A monitoring inspection was conducted on 04/08/2022 and a virtual record review was conducted on 04/07/2022. The inspection was conducted via Zoom. There were 34 children, ages 6 years-12 years, and 4 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 3 children?s records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and discussion with staff, the center failed to obtain a copy of a child abuse and neglect search and a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 2 contained documentation that Staff 2 had resided outside of the state of Virginia within the preceding five years. There was not a child abuse and neglect search nor a sex offender registry check from all of the states in which the individual had resided in the preceding five years available for review during the 04/07/2022 inspection. The documented hire date in the record for Staff 2 was 01/10/2022.
