Comments:
An unannounced on-site monitoring inspection was conducted on 09/22/2022 and a staff record review was done on 09/09/2022. There were 40 children, ages 5 years-10 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. The children were observed arriving to the program, having an afternoon snack, during homework time and going outside to play. A total of 5 children?s records and 4 staff records were reviewed. The inspector arrived to the center at 2:45 pm and departed at 4:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and discussion with staff, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence: The record for Staff 4 contained a documented hire date of 04/14//2022; the date on the employment eligibility letter was 04/15/2022.
Plan of Correction:
Will get fingerprints prior to hire.

Description:
Based on record review and discussion with staff, the center failed to obtain the findings from a search of the central registry within 30 days of employment.
Evidence: The record for Staff 4 contained a documented hire date 04/14/2022; the findings from the search of the central registry was dated 06/27/2022.

Will follow-up and document when not received in 30 days.

Description:
Based on record review, the center failed to ensure that staff shall submit documentation of a negative tuberculosis screening prior to coming into contact with children.
Evidence: 1. The record for Staff 3 contained a documented hire date 10/12/2021. The record for Staff 3 did not contain documentation of a negative tuberculosis screening. 2. The record for Staff 4 contained a documented hire date of 04/14/2022. The tuberculosis screening for Staff 4 was dated 04/20/2022.

Will have the staff member get one.

Based on observation, the center failed to ensure that children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals and snacks.
Evidence: The children were observed returning to normal activities and homework immediately following the afternoon snack. The children did not wash their hands after eating snack.

Will use handwipes after snack.

Description:
Based on observation and discussion with staff, the center failed to ensure that tables shall be sanitized before and after each use for feeding.
Evidence: The tables used for afternoon snack were not sanitized before or after the afternoon snack. Children were observed sitting at the tables upon arrival and before handwashing took place prior to the afternoon snack being served. Children were observed immediately resuming activities at the tables upon completing their snacks.
Plan of Correction:
The tables will be sanitized before and after the children eat.
