Comments:
An announced monitoring inspection, to review staff records and policy/procedures, was conducted on 05/19/2023. A total of 5 staff records and 5 applicant/agent records were reviewed. The inspector arrived at 10:00 am and departed at 12:40 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review, the center failed to obtain a copy of a child abuse and neglect search from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 1 contained documentation that Staff 1 has resided outside of the state of Virginia within the preceding five years. The documented hire date for Staff 1 was 03/10/2023. There was not a copy of a child abuse and neglect search from all of the states in which the individual had resided in the preceding five years available for review during the inspection. Staff 1 was observed working during the inspection.

Called the state to verify the process during the inspection. Will resubmit a request.

Description:
Based on observation and record review, the center failed to ensure that at least one staff in each classroom or are where children are present shall have a current certification in CPR and first aid.
Evidence: Staff 1 was the only staff member with a group of children outside on the playground on 05/08/2023. Staff 1 does not have a current certification in CPR and first aid.

Will get the staff member certified.

Description:
Based on observation and record review, the center failed to ensure that there shall be at least two staff members who have a current certification in CPR and first aid on the premises during the center?s hours of operation, on fieldtrips and whenever children are in care.
Evidence: Staff 4 was the only staff member with a current certification in CPR and first aid on-site from 2:40 pm until 3:20 pm on 05/08/2023.
Plan of Correction:
Will ensure that there is always 2 people on site that are certified in CPR and first aid.
