Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios, CPR certification, out of state searches, and accessing the department's website for notices, forms, and additional information. A copy of the new regulations was given to the director during this inspection.
Comments:
An unannounced monitoring inspection was conducted today, July 30, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 2:10pm and concluded at approximately 5:20pm. No children were in attendance at this location during this inspection. Administrative records, children's classrooms, restrooms, the outdoor playground, and common areas were inspected. See attached notice for the violation found during this inspection.

Based on the inspection of the building the center did not ensure that children are free of obvious hazards.
Evidence: During the inspection of the building, chipped paint was observed in the blue classroom in multiple areas near the doors and far wall. Also the gate to prevent children from accessing the upstairs was not secured to the wall, exposing the screws and allowing access to the upstairs area where various supplies are stored.
Plan of Correction:
Per administrator: I will have the room repainted and will have the gate repaired to prevent access.
