Comments: An unannounced complaint inspection was made on 11/1/19. A complaint was received from the online reporting system on 9/11/19 that there were forbidden actions, in general, used by staff. Four staff were interviewed and did not validate any of the allegations made in the complaint. The inspection was completed on 11/4/19 and sent via email to the director on that date.
