Comments:
An unannounced monitoring inspection was completed on 1/2/20. There were 29 children present in two groupings from seven months through nine years with seven staff supervising. Five children's records and five staff records were reviewed. No medications were at the center for any children in care.

The inspector arrived at the center at 10:15 am and departed at 2:30 pm.

The children were observed during free time in the gym, during movie time, during lunch, and during nap.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center failed to ensure that there was documentation that child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. The records for children #1, #3, and #5 did not have documentation of immunizations prior to the child attending. The first dates of attendance were documented in the children's records as #1 - 11/5/19; #3 - 6/6/19; #5 - 4/17/19. The immunization records were signed and dated by the physician on the following dates: #1 - 11/6/19; #3 - 7/5/19; #5 - 4/25/19.

Plan of Correction: The director will make sure children do not attend until documentation of immunizations is received.

Standard #: 22VAC40-185-320-B
Description: Based on observation, measurement, and interview, the center failed to ensure that the water in restroom areas had water that did not exceed 120 degrees Fahrenheit.

Evidence:
1. The temperature of the hot water was measured using a digital thermometer in five restroom area sinks. The temperature of the hot water in the sinks was: 124.9, 126.2 (in three sinks), and 124.9 degrees Fahrenheit. This exceeded the maximum temperature of 120 degrees Fahrenheit.

2. The director stated that a new hot water heater had been installed recently by the landlord and the temperature was not checked after this was done.

Plan of Correction: The director will contact the landlord to have the temperature on the hot water heater turned down.
