Comments:
An unannounced monitoring inspection, resulting from a self-reported incident received on 05/14/2024, was conducted on 05/20/2024. The inspector reviewed at total of 1 child?s record, 3 staff records, and interviewed staff. The inspector arrived at approximately 8:15 am and departed at approximately 9:50 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on observation, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence: 1.The height of the slide of the play structure measured approximately 36 inches. The resilient surfacing (mulch) fall zone measured approximately 6 feet in front of the slide chute. A total of 7 feet of resilient surfacing (mulch) fall zone is required in front of the slide chute. 2.The resilient surfacing (mulch) fall zone to the left of the slide measured approximate 4.5 feet. A total of 6 feet of resilient surfacing (mulch) fall zone is required underneath and surrounding equipment.
Plan of Correction:
Provider will obtain additional mulch.
