Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 06/01/2020 and concluded on 06/02/2020. The director was contacted by telephone to initiate the inspection. There were 41 children present and 11 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed three children's records and three staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on a review of three staff records and interview, the center did not ensure that one staff record contained all the required information.
Evidence: 1) The licensing inspector observed the sworn statement in the record for Staff #2, hired on 03/09/20, was dated 05/25/20. 2) Based on interviews and a written statement, the sworn statement for Staff #2 was signed on 03/09/20, but the original document was misplaced and not maintained as required. A sworn statement or affirmation is a background check required by the regulation entitled Background Checks for Licensed Child Day Centers and required to be in each staff record.
Plan of Correction:
Per the Center: "As stated in the description of violation, the sworn disclosure for staff number 2 was completed and signed on the date of hire 3/9/2020. This original document was misplaced and when the staff member completed a second sworn disclosure it was not properly backdated. Site administration was aware of staff number 2?s sworn testament and this testament was in compliance with licensing regulations. All staff have been retrained on file management and the administrative team is responsible to ongoing compliance."
