Comments:
An unannounced renewal inspection was conducted today between 10:10am and 11:55am. There were 5 children present, with 2 caregivers supervising within the supervision guidelines. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, nutrition, special care and emergencies. There was no medication on sight. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on the record review and interview, the provider did not review annually with the parent the emergency contact information to ensure the information was correct and obtain the parent's signed acknowledgement of the review. Evidence: 1. The last annual update for Child #1 was dated 6/1/22. 2. The last annual update for Child #3 was dated 7/25/22. 3. The provider acknowledged the review of the emergency contact information was not reviewed.

I will have the parents review the emergency information tonight and make any changes if needed.

Based on record review, a child's record did not contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Evidence: 1. The record for Child #3, did not have two designated persons to contact in case of an emergency.
