Comments:
An unannounced non-monitoring inspection was conducted on 11/29/2022 and 11/30/2022 regarding self-report received on 11/18/2022. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 3 staff records were reviewed. Interviews were conducted in conjunction with local Child Protective Services.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to ensure that Central Registry results were received by the end of the 30 days of employment. Evidence: 1. Staff #1 start date on file was 3/24/2022. 2. The record showed the central registry was sent on 4/21/2022. Staff #6 confirmed the central registry check had been sent within 7 days of employment.
Plan of Correction:
Management will send the CPS Background check out within the first week of employment.

Description:
Based on interviews, the center failed to provide needed help, comfort and support to a child. Evidence: 1. A relative of child #1 reported to the center that she observed, on the center?s camera system, Child #1 crying and being ignored by Staff #5. 2. Interview with Staff #4 confirmed that Child #1 was crying and ignored by Staff #5, as witnessed by Staff #4. 3. The CPS Representative stated that during an interview with Staff #5, Staff #5 admitted to being irritated with and ignoring Child #1.
Plan of Correction:
Providing additional training on Abuse and neglect by 12/14/2022 and offer additional breaks as needed.
