Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 05/28/2020. The director was contacted by telephone to initiate the inspection. There were 77 children present and 13 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the center did not ensure that one of four children had documentation of immunizations prior to attending the center.
Evidence: 1. Child #1 (DOE: 08/26/2019) had immunization records dated 10/01/2019. 2. Administration confirmed that the immunizations were obtained after enrollment.
Plan of Correction:
Going forward, all newly enrolled children will be required to turn in physicals and immunizations prior to starting care.

Evidence: 1. Child #1 (DOE: 08/26/2019) had record of a physical examination dated 10/01/2019. 2. Administration confirmed that the physical was obtained late.
