Comments:
A monitoring inspection was conducted on 05/15/2024. There were a total of 11 children (32 points) observed with 3 caregivers present. The children were engaged in the following: playing with toys, reading books with caregivers, and diapering procedures. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 3 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22.1-289.036-A
Description: Based on a review of caregiver records, the provider did not ensure background checks were repeated every 5 years after the first check.
Evidence:
Caregiver #2 had an expired central registry background check.

Plan of Correction: Caregiver #2 which is the one of the house members was away in the university and just came. Her new form has been sent and I will send you the respond one I receive it.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider did not ensure the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Unsafe conditions include chipped or peeling paint.
Evidence:
There was an area on the red wall by the entrance to the basement by the infant bouncer stations that was approximately 10 inches by 10 inches with peeling paint covered in plaster.
There was an area on the red wall in the hallway on the way to the changing station that was approximately 8 inches by 8 inches of peeling paint.
There was an area of peeling paint on the orange wall by the by basement door
There was an area of peeling paint on the green wall near the couch that was approximately 5 inches by 5 inches.

Plan of Correction: I will paint all peeling areas, however, the peeling occurred from the bouncer (jumper) friction against the wall during use. I repainted for so many times, but due for frequent use, it keeps happening.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the provider did not ensure caregivers washed hands at all required times.
Evidence:
Caregiver #1 was observed changing the diapers of several children. Caregiver #1 was observed washing their hands when they were finished changing all of the diapers.

Plan of Correction: I was wearing gloves on both hands and when i'd finish each child I changed into new gloves. Then when all changes were done I washed my hands fully, but I will wash my hands before and after.

Standard #: 8VAC20-800-680-B
Description: Based on observation, the provider did not ensure that children's hands were washed with liquid soap with warm running water during required times.
Evidence:
Caregiver #1 was observed changing several children's diapers without washing their hands after the diaper change.

Plan of Correction: When the inspector arrived at the daycare, the children had finished lunch, washed their hands, and were ready to move to the play area. I always ensure to wash their hands and I will wash them more. During inspection time I was changing infants on the changer but not older babies. I did not wash infants hands but I wiped them due age and size. I will follow the standards and wash their hands with soup and water.
