Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on November 13, 2020 and concluded on November 16, 2020. The director was contacted by telephone to initiate the inspection. There were 26 children present and six direct staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and interview, the center did not request annual parent confirmation that the required information in the child's record was up to date.
Evidence: 1. Child #1's, Child#2's, and Child #3's record did not contain documentation of an annual review. 2. Staff #1 stated that documentation was not available for review in Child #1's, Child #2's, or Child #3's record.
Plan of Correction:
Documentation of an annual review will be completed by all families and placed in the record.
