Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 29, 2020 and concluded on May 29, 2020. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 10 children present and 3 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on 1 of 2 records reviewed, licensee did not ensure children records had a copy of a physical examination within 30 days after the first day of attendance. Evidence: The record for child #1 ( Start date 5/23/2019) did not have documentation of a physical examination 30 days after the first day of attendance

Parent was notified about missing documents. Parent will bring it in.
