This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated and concluded on August 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present with 3 caregivers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of records, the provider failed to obtain documentation of a physical examination by the 30th day of attendance.
Evidence: Child #1's records did not contain documentation of a physical examination by the 30th day of attendance. The Child's first day of attendance was 4/5/2021.
Plan of Correction:
i will ensure that all children enrolled will have documentation of a physical by the 30th day of attendance.

I will contact OBI to find out where the central registry check is in the process.
